Headrest with cushion is an essential upgrade for long-distance touring and "expedition" style riding. On a recumbent trike, the neck muscles often fatigue because they are holding the head at an angle not common in upright cycling. The cushion is shaped to support the base of the skull (occipital area) rather than the neck itself, which prevents "head-bounce" when hitting bumps. Whether you prefer a "laid-back" aerodynamic position or a more upright "touring" stance, your eyes are level with the horizon without muscle tension.
